This questionnaire consists of 21 groups of statements. Please read each group of statements carefully.
Then pick out the one statement in each group that best describes the way you have been
feeling during the past two weeks, including today.
If several statements in the group seem to apply equally well, choose the one with the highest number for that group.
Be sure that you do not choose more than one statement for any group.

About the BDI-II
The Beck Depression Inventory-II (BDI-II) is a widely used 21-item self-report inventory measuring the severity of depression in adolescents and adults. Developed by Dr. Aaron T. Beck and colleagues, it assesses symptoms corresponding to criteria for diagnosing depressive disorders listed in the DSM-IV. The BDI-II is designed to identify the presence and severity of symptoms consistent with depression, not to formulate a clinical diagnosis.
Note: These score ranges are typical, but interpretations may vary. This tool does not provide a diagnosis.
If you have concerns about your mood or mental health, please consult a healthcare professional.
